3. AI Assistant — Scope and Limitations

Ascend Functional Medicine may use an AI-powered assistant ("AI Assistant") to help answer general questions, schedule appointments, and route inquiries by phone, text, or chat. By interacting with the AI Assistant, you acknowledge and agree to the following:

• Not a medical provider. The AI Assistant is an administrative and scheduling tool only. It is not a physician, nurse, or licensed healthcare provider, and it does not practice medicine.
• No medical advice or diagnosis. The AI Assistant does not provide medical advice, diagnosis, treatment recommendations, or interpretation of symptoms, test results, or medical records. Any general information it shares is for administrative or informational purposes only and is not a substitute for evaluation by a licensed healthcare provider.
• Emergencies. The AI Assistant is not equipped to handle medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
• No liability for AI Assistant output. To the fullest extent permitted by law, Ascend Functional Medicine disclaims liability for any decision made, or action taken or not taken, in reliance on information provided by the AI Assistant. The AI Assistant may occasionally provide incomplete, outdated, or inaccurate information, and any care-related decisions should be confirmed directly with our clinical staff.
• Verification by staff. Any appointment, prescription, billing, or care-related matter discussed with the AI Assistant is subject to review and confirmation by our human staff before being considered final.

4. Appointments and Cancellations

When you book an appointment, you agree to our cancellation policy. We require at least 24 hours' notice for cancellations. Late cancellations or no-shows may be subject to a fee.

5. Communications

By providing your contact information, you consent to receive communications from us, including emails and SMS messages related to your appointments, care, and inquiries. Message frequency varies. Message and data rates may apply. You can opt-out of SMS messages at any time by replying STOP.
